Please bring a referral from your GP or Specialist.
What do you need for your appointment
Please remember to bring the following to your appointment with Dr Riazi:
- Referral letter from GP or Specialist
- Medicare Card or DVA card number
- Private Health Insurance details (if applicable)
- Printed Radiology
- Pathology Results (if applicable)
- Recent x-rays, films, CDs or Radiology ID reference
- Correspondence from other treating specialists
What happens during your appointment
Most new patient appointments run for approximately 25 minutes. During this this time Dr Riazi will discuss you your:
- Medical history
- Injury, ailment details
- Results from any investigations
- Treatment options.
Dr Riazi will allow plenty of time with you to discuss your injury, treatment options and answer any questions you may have regarding your condition.
Please feel free to have any family members or friends present for your appointment.
Follow up consultation
A follow up consultation with Dr Riazi may be booked to view and discuss any further investigations or results regarding your treatment. We will make every effort to schedule this at your earliest possible convenience.
Fees
You can conveniently pay your outstanding account after your consultation by:
- Cash
- Credit card or;
- Eftpos.
A discount is applied for pensioners.
Private patients with Medicare
For private patients with Medicare, your consultation rebate will be claimed on your behalf.
Uninsured, WorkCover and TAC Patients – should be referred by their General Practitioner direct to Orthopaedic Outpatients, Box Hill Hospital (Eastern Health) for consultation and treatment.
Surgical fees – Private insurance patients
Where surgery is booked for private patients, surgical fees will be discussed at the time of booking a date. An estimate of fees, including item number/s, will be provided as soon as possible for patient to check health insurance cover and hospital paperwork is provided.
Uninsured patients
If the patient is uninsured and considering private treatment, then an estimate of fees for surgery encompassing surgeon, assistant, anaesthetist, hospital fees and any prostheses/equipment required, etc, will be forwarded to the patient, payable prior to admission. If they wish to proceed with public treatment, then they will be referred to Box Hill Hospital (Eastern Health).
